Termination by Member:

Members may cancel their membership by providing 30 days notice. All funds banked in account at time of cancellation will remain. All discounts will become null and void. Cancellation fees may apply as follows:

Radiant Glow: $150
Revive: $250
Rejuvenate: $375

Refunds:

No refunds will be provided for unused funds upon termination or cancellation. Unused funds will remain on the patient account as a credit.
